How to get re-entry in the register of pharmacists? If you have not paid your renewal fees in time your name stands removed from the register under section 33. Upon payment of the renewal fees along with the defaulter charges and submitting required documents your name can be re-entered in the register of pharmacists. You can download form 13 for re-entry along with identity slip and submit same to the office (in person) with following documents. Identity slip duly attested by a gazetted officer.
