Referrals come from a variety of sources, including professionals across statutory services and from the voluntary sector. Families can also self refer. We need to ensure that services we offer are suitable for the children and families that are referred. As part of the referral process, we contact the professionals involved in the child’s care, including their GP and consultants to determine their level of need. We provide palliative care and end of life care for children (from 0-19) with life shortening conditions and their families living in West Yorkshire. We focus on enhancing quality of life which includes physical comfort and wellbeing, as well as the emotional, social and spiritual aspect of care. We offer an inclusive service that embraces all faiths, beliefs and cultures. We provide palliative care and end of life care for children and young people and their familie. However in the area we cover there are more than 1,300 children who may need our support, so there is much more to do.
