Creening, monitoring, referral and follow up services help Community Rehabilitation Center consumers get the medical attention they need. Onsite Primary Health Care services include. Clients undergo a complete physical with lab work, which screens for chronic diseases such as diabetes, hypertension, heart disease, and kidney functioning.
