U of I Resources. The University of Iowa. This form is used before the subject is allowed to enter any scan room. Subjects must remain outside of the 5 gauss line until they have been screened by an MR technologist. The MRI Screening Sheet must be completed and signed by the subject. The Research Protocol Form is used to properly enter and run the desired protocol on the scanner. In addition, it provides instructions for data archiving and information about equipment needed for the study.
