We have a wonderful team of 7 Health Connectors, 3 Area Leads, 1 Volunteer Coordinator and scores of Health Champions. Work with patients 1:1 and help run the Self Management Groups and On Track Groups. Trains members of the public to become Community Health Champions. Community Health Champion training in June 2015 10.30-12.30 at Frome Medical Practice where we trained 13 Health Champions. Health Connections ‘Community Health Champions’ are members of the community who signposting friends, family, colleagues and neighbours to support in their own communities.
