The HouseCalls Program is participating in a component of the BRIDGES initiative through the Integrated Home-Based Primary Care (IHBPC) study. This study aims to improve access and build capacity for the care of home-bound seniors. The study’s co-principle investigators include Dr. Mark Nowaczynski (Clinical Director, House Calls), as well as Drs. Samir Sinha, Sabrina Akhtar, and Thuy-Nga (Tia) Pham. SPRINT Senior Care is the lead agency of HouseCalls. A not-for-profit program providing frail and homebound seniors with physician-led, interdisciplinary care at home. Under the clinical leadership of primary care physician Dr. Mark Nowaczynski, the integrated HouseCalls team includes two additional part-time physicians, a social worker, an occupational therapist, a physiotherapist, and a nurse practitioner, all of whom are supported by a team coordinator.
